Im sharing my story so you, unlike me, can be an INFORMED consumer if you chose to do business with Barnet-Dulaney-Perkins / American Vision Partners / Southwest Eye Center (they are all the same company). This is a completely factual story and you are welcome to contact me for more details: DR.

CARLOS DIAZ, DUE TO INADEQUATE SAFETY PROCEEDURES AT BARNET-DULANEY-PERKINS / AMERICAN VISION PARTNERS/ SOUTHWEST EYE CENTER AND PERHAPS HIS OWN NEGLIGNCE, IMPLANTED THE INCORRECT LENSES IN MY EYES. Although both Dr. Diaz and Barnet-Dulaney-Perkins / American Vision Partners / or Southwest Eye Center have admitted the error, they have done very little in to resolve the issue in a meaningful way. Their attitude seems to be: YEAH, WE MADE A COMPLETELY NEGLIGENT ERROR, IT WAS TOTALLY OUR FAULT, SO WHAT???

Here are the details: During consultation for cataract surgery - and after the hard sell from their surgical consultant (salesperson) - I was persuaded to pay over $5,000 extra for advanced lenses and a "superior procedure that that I was promised would provide the very best vision correction possible.

WHEN THE SURGERY WAS PERFORMED, THEY IMPLANTED THE WRONG LENSES! Both Dr. Diaz and an outside consultant agree that it would be very risky to redo the surgery. Now Im stuck for my entire lifetime with the consequences of their error.

Although his is incredibly distressing, THERE IS MORE: I HAVE OBTAINED EVIDENCE THAT THIS HAS HAPPENED TO OTHER PATIENTS BEFORE ME ANY YET, PROCEEDURES TO PREVENT FUTURE OCCURANCES WERE NEVER PUT INTO PLACE! Unbelievable, but 100% true. I have been assured procedures have NOW (under threat of a lawsuit) been corrected, Im still stuck with the results of their repeated earlier negligence.

Barnet-Dulaney-Perkins / American Vision Partners / or Southwest Eye Center treat their patients like a moneymaking commodity; this place is a factory. Do what is right for you, but since my disastrous experience, I have found that there are many SUPERIOUR OPTIONS FOR THE SAME PRICE.

FULL DISCLOSURE: A lawsuit is being prepared, since some companies just WONT do the right thing on their own. I will publicly share all documents I receive as part of discovery (where legal to do so) on a public website along with those folks that wished to contact me directly.

This business being investigated by the Arizona Medical Board. Will update once findings made. Estimated monetary loss to the patient is $50,000-150,000, not the $50 stated in the review.

Cash cow for practice!

My wife had cataract surgery with this practice. Big big mistake! They charge way too much and deliver way under. They recommended what new lens my wife needed for the cataract surgery, screwed it up so badly that now we are back, giving them more money, for glasses to cori the f'd up surgery. The Goodyear office now uses check in kiosks instead of face to face check in. Much less focus on patients and more focus on cramming more people through the door. I will tell everyone I meet to avoid this *** practice. Save your eyes and money and go elsewhere!!!

Poor service

When I called for my appointment, the gal took all my insurance information. I showed up today and after sitting for 45 minutes she called me up and asked for my referral, why didn't they tell me when I made my appointment? I won't be going back there., and office was packed, and I'm sure they were overbooked.

I the above mentioned patient has been a loyal patient of your Medical Practice for 10+ years. Between my Insurance companies and myself your practice has collected thousands and thousands of dollars on my behalf.

Im now truly disappointed to say the least! I have been what the AMA is legally termed Abandonment which is illegal.

Please see the statue:

Patient abandonment is a form of medical malpractice that occurs when a physician terminates the doctor-patient relationship without reasonable notice or a reasonable excuse, and fails to provide the patient with an opportunity to find a qualified replacement care provider

Every since of this statement applies to our relationship. I was notified via phone message on Friday March 4, 2022 that my appointment with Dr Berbos was being cancelled as he was not going to be available, period. I phone your office on Monday March 7th to reschedule and was told that Dr Berbos was moving out of state I then ask for an appointment with Dr Grant Moore whom I previously seen in Dr Berbos absence.

I was told the Surprise office would call me back. I waiting almost 2 weeks and no call back, I again phoned and was told that Ill send a message to the Surprise office no call back from Surprise office! Meantime my symptoms of blepharospasms are worsening and Im beginning to panic. I again phoned and again was told, Surprise will phone you!!!

And again, no call back. This went on for another couple of weeks and then I was rudely told if you dont like it find another Dr, WHAT!!!, finally after a minimum of 10 more calls, I finally got a young man from your office who made me an appointment with Dr Moore on June 27, 2022 at 3:50 pm. ( I told every operator I spoke to the necessity of my appointments due to the disease and I was becoming fearful to drive. The young man who set my appointment suggested that I call every Monday or Tuesday to check for cancellations.

On my call Tuesday May 16, w

2022 I was told I did not have an appointment for June 27, 2022.

WHAT!!!

I had called Southwest Eye Center and went to an appointment but then realized they did not do the injections.

Now, to make myself clear, has Perkins Barney Delaney abandoned me as a patient with no cause or reason up to this date or is there some sort of a major breakdown of inadequate communication.

To be clear, I am unable to drive and Im spending a fortune on Uber transporting me to my husband hospice center! Im stumbling around the house and frightened of falling.

Im asking you to please resolve this issue and get me into see Dr Moore asap as I cannot locate another Oculoplastic Surgeon that treats my condition with Botox in Maricopa.

I would certainly agree that the both of us would not like this to go any further.

Coordinator asked me give me a chance. She was a gold mine if hustle, follow up and timeliness

Processed to have successful cataract removed.

Next day follow up an hour after initial girl checked me in. 6 walk talk doing nothing in the wait area until Dr. Calls y. They treat u like a cattle cart waiting for slaughter.

Sorry but no one scheduled queue time..big bus gold mine but we pay the price waiting longer than er.quue customers further apart not cluttered like sheepschedule w common sense not stuffing bodies. First coordinator dumped me said call billing who never responded ie mayaha..sicschedule w some common sensefull time queue study

Overcharged / Non Disclosure

Diabetic exam paid for by my insurance 100%. However, they charged me for another service without an up front disclosure that they were performing the service and was charged additional fees which included a 30.00 copay. My eye exam was for diabetes at 100% and no copay. Additional service performed was not brought to my attention by the doctor. Barnett said I mentioned something about floaters and that is what incurred additional charges and my copay. Never mentioned by the doctor or office staff. How does this business get away without disclosure(s) at time of services? From other reviews I have read, this is not an isolated event. Where are the medical/vision regulators? Never again with this monopoly of eye care in the Phoenix area. There appears to be no compliance monitoring.

I was scheduled for surgery 3/15/22 by dr funke. I was notified humana was pending approval.But when i called 3/21 i was told by humana dr funke cancelled the surgery w no notice to me. Billing clerk makaya wont return any explanation or try t reschedle my needed surgery. I have 25 hours ctraveling waiting in ,testibg waiting history physivallines lasic that was worthless all in the last6 months

No one rescheduled my eye surgery or even followed up. 6 mo 25 hours of travel exams lines and set dat and they don't even follow Humana said they called and cancelled on 3/21 and never told me other than pending and does not meet criteria AFTER 6 months , meetings waiting history,MD scans testing. Had cataract surguryin December-what a freaking disaster. Between getting it schedule and a number of run around by both

The procedure for the first eye was done a week late because the clinic did not follow thru with scheduling the operation correctly. When I got in for the first eye - the sedation was not adequate and I ended up waking halfway thru the procedure.

Nurse and doctor holing my arms down to keep me from causing further issues. A week later I allowed the second eye to be done. This time I remained sedated through the entire procedure.

After the eyes were done, I had issues with glare that did not allow me to open them in the daylight. After visiting the clinic for a followup - was given some drops and sent away.

Meanwhile, the vision issues with light were not getting any better. Went to an ER, and they were not any assistance.

I finally checked with one of my other doctors - ended up in the hospital for several days. I was anemic, it turned out. I can see again and no thanks to the doctor that I was supposed to depend on.

She refuses to accept any responsibility for the extremely low quality of the services provided.

My eyes are still not close to anticipated condition. I have not found an alternate sevice provider to replace these sons of a female dog.

Dishonest Business Practices

One of my family members went to one of their clinics as well. Charged them a copay for the exam and when they went to go get glasses they were told that they would have to pay another 51.00 for the prescription to be handed to them. BTW this is in violation of a FTC statute called the Eye Glass Rule aka. Contact Lens Rule. It is also a violation of their contract with the insurance. Prescriptions are tied to office visits and a Dr. Cannot withhold a Rx for additional payment when it is part of the exam for which your insurance paid. So. I reported them to the FTÇ , Medicare and Medicaid for Fraud and abuse since you also cannot charge Medicare and then bill a patient additional fees for covered services. Please dont go here. There are plenty of other honest businesses out there.

Preferred solution: I would like them to answer to Medicare/FTC regarding violations of Federal Statutes and unethical business practices. Luckily my in laws knew not to pay additional fees.

User's recommendation: I don’t recommend doing any kind of business here. There are plenty of other eye physicians in the valley who will treat you well and in accordance to insurance policies, FTC Policies, and Ethically.

We went there 4 times still no surgery blame us for primary care not getting paper to them we got paper told us not to fax it but to bring it in which is 170 mile round trip with my husband who has muscular dystrophy did not want me to be with him he has trouble walking and understanding them about what their doing more worried about paperwork then patients

Charged for an office when my insurance company says there is no charge for a diabetic eye exam

I was charged $40 for a diabetic eye exam, according to my insurance there is no charge for this eye exam, also they wanted to charge $52 for a copy of my new eye glass prescription. My insurance company called the office and told them there should have been no charge but the office still refuses to give me a refund.

Their explanation for the $52 charge was it is not for the prescription but for a test not covered by insurance. Seth in customer service told me the charge was waived but they couldn't mail me the prescription I would have to go back to the office to get it. When I talked to the office manager Paula she says the charged was not waived and it as still $52 for the prescription.

The only tests they did was a normal eye exams and some eye drops. These people are theives.Don't use them

After my cataract surgery, I unhappy with distance vision. Dr. Olivetti said I could talk with a Lasik rep and someone would call me. When checking out, I asked if there was a note on my file to receive a call from Lasik. Was told yes. Two and a half weeks later still no call. I called on a Monday and was told someone would call me. I called again two days later and still no call from Lasik. They don't care about their customes. So sad.
